Important Information About Obtaining Medical Records

Fee Schedule for Copies (We accept cash only, no checks or credit/debit cards)

• $15.00 handling fee for all paper records (records from 2002 to present)
• $20.00 handling fee for all electronic records (records prior to 2002)
• $1.00 per page for pages 1-25
• $.50 per page for pages 26-500
• $.25 per page thereafter

Records are retained for ten years as per Louisiana law.

Huey P. Long/Infostat will provide your primary care physician with an abstract of your medical records if you provide us with your physician's contact information.  Patients with proof of appointment or a letter from their primary care physician (on the physician's letterhead) will be provided with the most recent information to hand deliver to their physician.

We will not release third party information. Records from other facilities must be obtained from the facility at which they were created.

Documents you will need to obtain medical records:

If you are the patient:

• Driver's license or picture id
• Proof of appointment, if records are being obtained for continuity of care

If you are not the patient:

• Driver's license or picture id
• Written consent in the form of a HIPAA compliant authorization from the patient
• A copy of the patient's driver's license or picture id

If the patient is a minor:

• Driver's license or picture id
• Proof of guardianship

If the patient is deceased:

• Driver's license or picture id
• Copy of the patients death certificate or obituary from the newspaper (funeral home programs are not accepted)

Operating hours:

We are open Monday through Friday from 8:00 a.m. to 4:30 p.m.  We do not close for lunch.
