Please bring the following:

  • Picture identification
  • All insurance/referral information
  • Required deposit, if applicable
  • Medications - Prescription bottles and over-the counter medications

Non-Louisiana Residents: You may be required to pay a minimum deposit of $15.00/clinic visit and may be responsible for total bill of services rendered.
