Prepare for a Hospital Stay

The following documents are available for review, printing and completion prior to your hospital visit.  You may bring these completed forms with you to your appointment.

What to Bring for Your Hospital Stay
As closet space is limited, please bring only a few items for your personal use:

• Picture ID
• All Insurance Information
• Required Referral letter
• Any required co-payments/deposits (LSUHSC Shreveport accepts Visa and Mastercard)
• Current proof of income (for patients requesting free care classification)
• LSU Patient ID card
• Pajamas or a nightgown (the hospital provides patient gowns)
• Dressing gown, slippers, underwear
• Toiletries such as a comb, a hairbrush, a toothbrush and toothpaste, soap, deodorant, shampoo, a shaving kit, etc.
• All personal articles should be labeled with your name.

Optional Items

Battery-operated equipment, such as a razor, lamp or radio (with earphones) is allowed. Electric hairdryers (less than 800 Watts) or curling irons are permitted but you should not use them while in bed or alone in the room. You may bring a small amount of money for personal needs such as newspapers, snacks, etc.

For safety reasons, please leave the following items at home:

• Butane or other gas-powered equipment
• Electrical appliances
• Jewelry
• Appliances such as a coffee maker, a fan, a humidifier, an electric hairdryer that uses more than 800 Watts
• Large sums of money, credit cards or valuable papers. If this is not possible, safety deposit boxes are available through University Police (security). LSUHSC Shreveport assumes no responsibility for any theft, loss or damage if you do not use the safety deposit boxes.

Medication

• Medications - Prescription bottles and over-the counter medications
• If your physician asks you to bring your medication that you are currently taking (including prescription drugs, over the counter drugs and herbal products) give it to the nurse or physician who admits you.
• A relative or friend may then return your medication to your home.
• If this is not possible, the medication will be kept for you until you leave the hospital.
• Remember to ask for your medication when you are informed of your discharge. Unless directed by your physician, pharmacist or nurse, do not take any of your own medication while in the hospital.

Dentures

If you wear dentures and remove them at night or have to remove them for surgery, please ensure that a family member keeps them for you and don't leave them in your hospital room unattended. Dentures can be picked up accidentally and thrown out when a room is being cleaned. Bring a container if you have one.
